Track and Review Signed Waivers and Membership Agreements

INTRODUCTION

Feature Summary: Generate a report that shows all signed waivers and membership agreements for a selected date range.

Use Cases: View signed waivers for specific membership types or all memberships over a defined period. Use this report to confirm compliance, audit member records, or prepare for insurance reviews.


PREREQUISITES

To access this report, the Electronic Waivers & Membership Agreements additional feature must be enabled.


SETUP REQUIREMENTS

  1. Log in to the admin dashboard.

  2. Click REPORTS > WAIVER(S).


Report Criteria

  1. Membership Types – Filter the report by specific Membership Types to see only waivers or agreements signed by Members with those memberships.

  2. Date Signed From – Set the earliest signing date to include in the report.

  3. Date Signed To – Set the latest signing date to include in the report.

  4. Waiver(s) / Agreements – Select specific waivers or membership agreements to include in the report.

Scroll down to the Setup Report Output heading. The table shows all possible columns that could be included in the report (based on the enabled filters) and the system setup. Check or uncheck boxes to add or exclude report columns.


Viewing the Report

Click the Run Report button (at the top or bottom of the page). A new tab opens with the report in a table format. Click a button to export the report (.xlsx or PDF).

To filter table data, click the filter icon, select an operator (e.g. is equal to), and enter a value. To change the sort order, go to a column heading and change the direction of the arrow.

Use the Email Players button to message the players included in the report. The Push Notifications button will also be available if this additional feature is enabled.


Saving a Report

Click the Save Report button to keep the report on the platform. Enter a report name and select a Visibility option.

  • Private means only the report creator can access this filter.

  • Shared means all users with access to this report type can view and run this filter.

The next time you access the report, use the Saved Reports button to load your saved report settings.
